I understand that the trainer/therapist is providing Strength & Conditioning Programme and/or Sports Therapy^ (the “Programme”) within their scope of practice as defined by the Australia Strength and Conditioning Association (ASCA) and Singapore Sports Institute (SSI).

I hereby consent to the trainer/therapist providing treatment to me under the Programme, including such assessments, examinations and techniques, which may be recommended by the trainer/therapist.

I acknowledge that the trainer/therapist is not a physician and does not diagnose illness or disease or any other physical or mental disorder. I clearly understand that the Programme is not a substitute for a medical examination or medical treatment. It is recommended that I attend my personal physician for any ailments that I may be experiencing. I acknowledge that no assurance or guarantee has been provided to me as to the results of any treatment or assessment offered to me under the Programme.

I acknowledge that with any treatment there can be risks, including (but not limited to) reasonably foreseeable ones which have been explained to me as well as those which may be inherent in the Programme and cannot be eliminated or reduced, and I fully assume all these risks. My participation in the Programme is voluntary.

I hereby release and discharge the trainer/therapist from all claims, liabilities, losses, suits or expenses (including costs and reasonable legal fees), made or brought by anyone, arising out of any injury, damage, death or other loss in any manner connected with my participation in the Programme and acceptance of the treatment thereunder.

I authorize the trainer/therapist to release or obtain information pertaining to my condition(s) and/or treatment to/from my other caregivers or third-party payers^^, as well as where required by law or regulations. Please be assured that ProCoach does not indiscriminately disclose client information.

I, hereby grant ProCoach permission to take photographs and videos of myself to publish those contents for any lawful purposes, including, but not limited to their website, social media accounts and promotional materials, either digital or in print.

I, the individual listed have attained 18 years of age, and am accepting and agreeing to this Informed Consent on my own behalf, and/or on the behalf of any listed participants under the age of 18 (“child/ward”) as, or as duly authorised agent on behalf of, their parent or legal guardian.

I understand that at any time I may withdraw my consent and treatment will be stopped, but without prejudice to the binding nature of this consent in respect of all treatment provided to me prior to such consent withdrawal.
